AFTERCARE SERVICE ARE RECRUITING FOR THE FOLLOWING POSITION;

PROJECT WORKER - SOCIAL CARE LEADER

This is an exciting opportunity to work with a number of State and
Voluntary organisations using a partnership approach on a project
funded through the Dormant Accounts Fund. This project focuses on
developing and improving pathways and links to services who support
young adults with a care history in a planned and structured way and
to provide direct supports to the young people involved in the
project. The focus is on improving outcomes for these young adults in
the areas such as; Accommodation, Health, and Education.

LOCATION: Cork

REPORTING TO: A Dedicated Management team made up of voluntary and
statutory agencies and supervised by appropriate Tusla personnel.

TERMS: Twelve month Fixed-Term Contract – *
hours per week - Flexibility will be required to accommodate the
service provision. Health sector consolidated salary scales apply.

OBJECTIVE: (1)To maintain and as needed implement new pathways to
service providers such as accommodation, mental health, and disability
(2) Provide direct supports for young adults as they transition from
Aftercare.

CORE DUTIES AND RESPONSIBILITIES:

* Further develop and as needed establish new links with networks
that support young adults who present with complex case presentations,
particularly those with disability diagnosis, trauma related issues,
mental health challenges and/or a combination of all of these.
* To work and promote a toolkit which has been developed in phase
one of the project
* To provide direct support to young people and supporting agencies.
* To report to and undertake duties as deemed necessary by the
Management team.

APPLICANTS ARE REQUIRED TO HAVE:

* A Bachelor of Arts in Applied Social Studies or equivalent.
* A minimum of 3 Years’ experience working in a community and/or
residential setting, involving interagency working and including
experience of the challenges faced by care-leavers.
* Experience in operating administration systems, recording
requirements and completing reports.
* Access to a car and hold a full driving license.
